Registration Fee: Each patient pays a one-time registration fee as outlined in the chart below. If you allow your monthly payment to lapse, you will be allowed to rejoin the program if space is available. In that instance, a $150 reactivation fee will be required.
Basic Monthly Fee Schedule: The basic monthly fee schedule is outlined in the chart below. It will be billed monthly to your credit or debit card on the first day of each month beginning the month following enrollment in the Program.
